The iconic two-handed sword, The Grandfather, and the all-powerful Shako (also known as the Harlequin Crest) make their return, along with four other ultra-rare unique items. Despite their extremely low drop rates, there is a method that can increase the likelihood of obtaining one of these six rarest Diablo 4 items. However, it is important to keep your expectations in check.
How To Obtain The Rarest Diablo 4 Unique Items
Players who have reached World Tier 3 and 4 are likely to have a substantial number of legendaries and unique items. However, the drop rates of these items are not as random as one might assume. It is possible to target specific enemy types for different drops. For instance, Goatmen have a slightly higher chance of dropping Totems, which are useful for Druids searching for Druid-specific Aspects. Unfortunately, this targeting does not apply to the six rarest Diablo 4 items.
There is only one way to obtain these rare unique items. Lead Class Designer Adam Jackson pointed out that players must be at least Level 85 in Nightmare difficulty (World Tier 4) to have a chance of obtaining these items. These six rare unique items will not drop for lower-level players. Players must simply complete content that rewards unique items, such as Helltide Mystery Chests, Nightmare Dungeons, and other endgame activities.

The first Harlequin Crest (Shako) was found on June 11, 2023, approximately a week and a half after the game’s early access launch. The first drop of The Grandfather came on June 28, 2023, nearly a month after the game’s release.
